Sandy Springs: How to Choose the Right ‘Cosmetic’ Dentist

Over the past decade there has been a dramatic appearance of ‘cosmetic dentists’ to the degree that almost every dentist is now a cosmetic dentist. Why? Because there is no such thing as a cosmetic dentist and therefore it is unregulated.  Because the term ‘cosmetic’ is unregulated there is no shortage of dentists pressing the envelope of ethics.

So how do choose the right dentist?

1. Make sure your ‘cosmetic’ dentist is accredited by the American Dental Association and not just a member.  This would be a prosthodontic trained dentist, which is a 3 year post-doctoral dental degree specializing in aesthetic and functional dentistry.
2. Don’t be fooled by all of the organizations that provide unaccredited certifications.  The only accredited specialties are those approved by the American Dental Association and cosmetic dentistry is not an approved credential.
3. Select a prosthodontist who performs cosmetic dentistry every day.  Creating the right smile is more of an art than a science.
4. Look for real reviews and a history of longevity within the field of prosthodontics.
5. Request a consult and ask to see before and after results.
6. Finally, trust your instincts.

A prosthodontist will have a significant amount of experience in complex cases.  According to Forbes Magazine the prosthodontist is the rarest dental specialty in the United States, making up less than 2% of the dentists.

So for the right smile make sure you have the right dentist, a prosthodontist.

If you are in need of tooth restoration, or if you are unhappy with the way your smile looks, your first inclination may be to head to a cosmetic dentist. Technically, there is no such thing.  Instead, it may be wiser to head to a prosthodontic dentist.

Most patients, in fact, almost all patients have never heard of a prosthodontic dentist. However, these dentists are highly regarded and certified by the American Dental Association (ADA) as what you might term “a cosmetic dentist”.

Cosmetic dentistry is NOT a recognized ADA specialty.

This means that dental schools instruct dentists in techniques, methods, and specialized training in prosthodontics—not in cosmetic dentistry.

Prosthodontic dentists  are post-graduate, university trained in certain types of procedures that are used in cosmetic dentistry.

What’s more, prosthodontic dentists receive an additional three years of post-dental school training.  Of the 200,000 dentists in the United States, less than 2% are trained prosthodontists.  According to Forbes Magazine, prosthodontics is the rarest dental specialty in the country. There are only 560 of these dental specialists who are devoted to restoring and replacing teeth.

During their extra three years of training, prosthodontic dentists study anatomy of the head, neck, and mouth as well as esthetics (the cosmetic part of performing procedures). These dentists are trained to be experts in the field of aesthetic dentistry.  They are commonly considered the quarterbacks of implant dentistry.

Many prosthodontic dentists also help patients with maxillofacial procedures. Maxillofacial procedures are ones in which acquired and congenital defects are treated. These can be any defects of the head and neck due to trauma or injury, or they could be defects that were present at birth. However, in order to be licensed to practice maxillofacial procedures, a dentist must complete an additional year of training. This is an extra year after the three extra years required for becoming a prosthodontic dentist.

There are two types of dentistry today.  One is traditional dentistry targeting the oral hygiene and prevention of oral diseases, tooth decay, etc., by diagnosis and routine oral treatment.  Its focus is maintenance oriented and a percentage which is covered by insurance.

The other and most recent is cosmetic dentistry, aiming to improve the appearance of your teeth and smile.  And in some instances dental procedures are implemented to improve function, which may have the tertiary effect of improving your smile, i.e. an implant.  Thus, general oral diseases and related health issues are treated by general dental practitioners during the routine care of your oral health.  Whereas cosmetic dentistry should be handled by a dental specialist, whether it is a prosthodontist, orthodontist or an oral surgeon/periodontist in conjunction with a prosthodontist.

Cosmetic dentistry, while it is not a recognized dental specialty, has risen to an art form in the field of dentistry.  When applied in conjunction with the restorative benefits of general dentistry like dental fillings fall within the domain of general dentistry but crosses over into cosmetics when the materials of the restoration serve two purposes; the filling itself and the appearance of the filling.  Thus a composite dental filling is covered by cosmetic dentistry to retain your look and smile as usual, but avoids the unsightliness of an amalgam (mercury filling).

There are many dental clinics and general practitioners providing a myriad of cosmetic dentistry procedures.  Before opting for treatment, one needs to take care in selecting the right type of dentist with investigation that should include references, reviews, training and the skill set for the treatment as well as the particular procedure and protocol to be followed.  In metro cities like Atlanta one can find many dentists who purport to be trained (or have trained themselves) as cosmetic dentists, which is a self-proclaimed non-existent specialty.  These dental clinics and practitioners are providing general dentistry as well as cosmetic dentistry that range from performing root canal treatment, periodontal treatment, children’s dentistry, teeth whitening, bleaching and advanced cosmetic dentistry such as the placing of implants.  Purported cosmetic dentists providing various cosmetic procedures as well as surgeries like full mouth rehabilitation, tooth laminates, tooth veneers, crowns, bridges, etc., can make a mess of your mouth if you’re not careful.

Prosthodontic dentists are with the passage of time gaining popular recognition as patients become more familiar with the advances in dentistry and how they can enhance their everyday lives. Dentist Dunwoody: Teeth Whitening

More and more the esthetics of teeth are of great importance to patients, including tooth color.  The color of the teeth is influenced by a combination of their natural color and the presence of any external stains that may form on the tooth surface.   The type of natural stain and initial tooth color can play a significant role in the ultimate outcome of tooth bleaching.  Where oral health care used to be the primary function of a dentist, in the past 10 years the importance of tooth whitening for patients has seen a dramatic rise in the number of tooth whitening products and procedures to come onto the marketplace.  The relationship of the doctor and patient has changed.  And the demand by the consumer to kind of cookie cutter the results of esthetic dentistry is supplanting the traditional approach to oral health care.

There are a number of approaches to measuring tooth color changes following tooth whitening; each with their own advantages and disadvantages; and given the consumer’s demand for continued esthetic emphasis, tooth whitening will continue to demand further research and perfection.  The key factors that affect tooth whitening efficacy by peroxide containing products are a function of peroxide concentration, to a limited degree, light exposure and the allocation of application time.  In general, higher concentrations provide results faster than lower concentrations. While lower concentrations approach a similar degree of results as higher concentrations with extended treatment times, what appears to be a demand for immediate gratification by the consumer apparently drives a propensity to develop systems that deliver higher concentrations of peroxide with shorter application time periods.

Dentist Dunwoody: The future of Implant Dentistry is now.

Dental implants are stronger and more durable than their restorative counterparts such as bridges, partials and dentures.  And from an esthetic standpoint, they look and feel more natural, some of which is due to the progress made in their biocompatible development.  Implants offer the patient a permanent solution to tooth and continued bone loss.

Additionally, implants may be used in conjunction with other restorative procedures for maximum effectiveness in that a single implant may serve to support a dental crown replacing a single missing tooth. Implants can also be used to support a dental bridge for the replacement of multiple missing teeth, and can be used with complete dentures, both standard and mini implants to increase stability and preserve bone structure.

Procedural advancements mean that a larger population than ever before are finding themselves candidates for dental implants.  While application and candidacy for implantation varies on a case by case basis, meaning that your dentist needs to determine the viability of implants as it applies to your actual bone structure.   Keep in mind, a general dentist may perform the crown and bridge placement that is associated with implant restorations, prosthodontists are the ADA sanctioned specialists who have received post-doctorate training are most often your best bet for the successful completion of this type of technique sensitive procedure.  Dr. Scheinfeld’s daughter, Dr. Bock has been trained to place implant through her implant residency in Birmingham Alabama.  If we can ever be of help please don’t hesitate to call or comment so we can answer your questions.

Dentist Buckhead: The Mercury Filling Controversy

Whenever I hear a patient ask about amalgam restorations (usually referred to as mercury fillings), I wonder why this question refuses to go away.  For decades, amalgams have been considered the primary restorative material for posterior teeth (the ones in the back) because of their long time success.  After enormous amounts of study the World Health Organization, the FDI World Dental Federation, and the American Dental Association continue to endorse the use of amalgam to restore teeth.   Yet it continues to be demonized by the public, in particular in urban legends over the internet.

As a result the trend is towards the less and less use of amalgams and the greater use of composite resins to restore posterior teeth.  One of our consultants, Tom Limoli of Limoli and Associates notes that US third-party payment data reflects that 65% of direct posterior restorations last year were resin-based composite, while 35% were amalgam.  So regardless of the empirical evidence to the contrary, the patient pool is demanding composite restorations.

Every dentist will need an alternative material to use in the restoration of posterior teeth as this trend continues.  The challenge for the dentist is that composite resins only have a life expectancy of 5 to 8 years.  Given the patient’s desire to be rid of the potential or theoretical health hazards that have been formulated in recent years, amalgam restorations will ultimately be eliminated by the slow and natural death of attrition.  While amalgam has been the material of choice for decades and still remains the primary source of teaching in dental schools today, it may not be in the future.  Given the patient demand for composite restorations and what appears to be the dentist’s propensity to capitulate, we are going to need a better solution to posterior restorations if we want to achieve the same longevity that is achieved through amalgam restorations.

The internet has continued to create an uncertainty on the part of the lay public about amalgams similar to the controversy that surrounds cell phones and brain tumors.  It is interesting to note the dilemma faced by dentists today, when we know that amalgam is the better choice for the restoration, but the popular demand or path of least resistance is a composite restoration.  This disconnect creates some interesting food for thought.[1]
